Liposuction primarily removes fat, reducing the volume of fatty tissue. However, it is not sufficient on its own to treat gynecomastia caused by breast (glandular) tissue. This gland tissue can be firm or more fibrous, and it usually needs to be surgically removed to achieve good results in male breast reduction.

Modern liposuction techniques, such as ultrasonic, tumescent, and laser liposuction, are very effective at removing fat. However, they cannot remove glandular breast tissue, which is often the main cause of true gynecomastia.
Some doctors prefer a cautious approach during surgery, because removing too much tissue can be harder to fix than removing too little. As a result, a small amount of leftover breast tissue may remain. The treatment depends on what caused the issue. In many cases, fat grafting can be used to fill the hollow area and create a smoother chest shape. If the skin has become stuck to the muscle, revision surgery may be needed to release it. Sometimes, a combination of techniques is used to achieve the best result.

Swelling can take up to a year to fully settle, and scar tissue continues to soften over time. It is generally recommended to wait at least four months after the initial surgery before considering revision.
A haematoma is a collection of blood under the skin. A seroma is a collection of fluid. Both are possible complications after surgery. Haematomas may cause pain, bruising, and swelling, and sometimes require surgical drainage.
Scar tissue is a normal part of healing and may feel firm at first. It can take several months to soften. Massage can help improve it, and in some cases, steroid injections may be used to reduce firmness and improve appearance.

Scar revision is rarely needed, as there are often other ways to improve scar appearance. If required, it is usually performed under local anaesthesia.
Recovery is similar to the initial procedure. Most patients can return to normal activities within about one week, but should avoid strenuous activity and heavy lifting for 4–6 weeks. Swelling and bruising usually improve within 4–6 weeks. Temporary numbness may last a few months, with full sensation returning within up to a year.
