Periareolar top surgery is a masculinising chest surgery technique designed to create a flat, masculine chest contour with minimal visible scarring. It is most appropriate for transgender men and non-binary individuals with smaller chest sizes and good skin elasticity. The procedure involves a circumferential incision around the outer edge of the areola, allowing breast tissue to be removed while preserving nipple sensation and natural nipple position.


Periareolar top surgery is performed by Dr Spiros Vlachos and Mr Andreas Shiatis at our CQC-regulated Baker Street clinic. How Does Periareolar Top Surgery Work?


Periareolar top surgery uses a circular incision placed precisely around the outer margin of the areola to access and remove the underlying breast tissue. The areola can be reduced in size and repositioned as part of the same procedure. Once the breast tissue is removed, the surrounding skin is carefully adjusted and secured to fit the new chest contourproducing a flat, masculine result with scarring confined to the natural colour transition at the areola border.


One of the primary advantages of the periareolar technique is that the nipple-areola complex remains attached throughout the procedure. The pedicle is preserved, maintaining the blood supply and nerve connections to the nipple — this is why periareolar top surgery is associated with better nipple sensation outcomes than techniques involving free nipple grafting.

Is Periareolar Top Surgery Right for You?


Periareolar top surgery is most appropriate for patients with smaller chest sizes and good skin elasticity. These factors are essential for the technique to achieve a flat, well-contoured result without additional incisions across the chest. Your suitability is confirmed at a face-to-face consultation with your surgeon.


Suitable candidates typically:


Patients with larger chests, poor skin elasticity, or significant ptosis are unlikely to achieve a satisfactory result with the periareolar technique alone. In these cases your surgeon will recommend double incision top surgery or inverted T top surgery as more appropriate alternatives.


A mandatory two-week cooling-off period applies from the date of consent to all surgical procedures at Centre for Surgery.


Periareolar Top Surgery Recovery


Periareolar top surgery is performed as a day case under TIVA. You will need a responsible adult to take you home on the day and stay with you for the first 24 hours.


A compression binder must be worn for six weeks after surgery. It reduces swelling, supports the healing tissue, minimises scarring, and maintains the new chest contour. Wear it as instructedremoving only to wash.


Most patients can return to desk-based work within one week. Avoid strenuous exercise and heavy lifting for six weeks. Light walking from day three or four is encouraged to promote circulation.


A wound check is included at seven to ten days. A surgeon review is included at six weeks. A three-month assessment is included as part of your surgical package. 24/7 clinical support is available for the first 48 hours after surgery.


Most swelling resolves within three to four weeks. Final chest contour is visible at three months. Periareolar scars are positioned at the areola margin and will continue to fade over 12 to 18 months.

Potential Risks of Periareolar Top Surgery


Periareolar top surgery has a favourable risk profile when performed on appropriate candidates. All risks are discussed in full at consultation.


Periareolar top surgery preserves the nipple pedicle, maintaining blood supply and nerve connections. Most patients retain partial to full nipple sensation. Some experience temporary numbness in the first weeks that improves as nerve recovery progresses. A small minority experience permanent reduction in sensitivity.


Reduced sensation across the chest skin is common in the first three to six months. Most patients regain partial chest sensation within 12 months.


The incision is placed at the natural colour transition at the outer margin of the areola, where it is well concealed once healed. Initial scarring may appear pink and slightly raised; it progressively flattens and fades over 12 to 18 months.


Specific scar risks for the periareolar technique include areola distortion or stretching — if skin redundancy is greater than the technique can accommodate, the areola may heal with an irregular or stretched outline. This risk is reduced by appropriate patient selectionperiareolar is restricted to smaller chests (typically B cup or smaller) with good skin elasticity.
